2017 Membership Form & Waiver

Authorization to Transport

All Applicants must submit a completed Membership Application Form a Waiver Form and remit fees PRIOR to taking the Club Level Safety Course. You may remit electronically, by mail or in person to our Secretary Treasurer or Membership Chair.

Please allow two weeks for processing.

We will not accept new Membership Applications, Membership Renewals, Waiver Forms or payments in our donation boxes.

Membership Renewals must be remitted by September 30th each year. There are no exceptions to this rule.
